What you will learn

  • Explain what a root cause is and why repeatedly asking why a problem occurred removes blame from individuals and prevents recurrence
  • Describe the steps of the 5 Whys, including grounding each answer in what actually happened, knowing when to stop, and monitoring the measures after a countermeasure is in place
  • Identify the benefits and risks of problem-solving as a group, including how it encourages responsibility and prevents alienation, and what to say when one team member dismisses another's idea
  • Describe methods for narrowing a list of options, including the difference between a mind map and a decision tree, and constraints such as technical feasibility
